From CSW to Care Coordinator: Fatoumata Niare's Journey of Comprehensive Care


Fatoumata Niare’s journey from a Community Support Worker (CSW) to her current role as a Care Coordinator in our Northeast office shows her deep dedication to supporting individuals at every level of their wellness journey. Starting as a CSW, Fatoumata was drawn to the opportunity to connect directly with those in need, offering day-to-day outreach, guidance, and building rapport with consumers. Now, as a Care Coordinator, she has elevated her role, setting up treatment plans, creating wellness schedules, and ensuring that each person receives the personalized care they deserve. 


“What drew me to become a care coordinator was my passion for helping others and making a meaningful impact on their lives," Fatoumata shares. Her background as a CSW gives her a unique perspective—understanding firsthand the struggles individuals face and the small actions that can make a significant difference. “One consumer approached me to express his gratitude, saying, ‘You saved my life, and you don’t even know it.’ That moment reinforced how important it is to offer a judgment-free space for people to feel seen.”

 

Now, Fatoumata combines the hands-on experience from her CSW days with the strategic care planning required as a coordinator. The transition has allowed her to support individuals not only in their day-to-day challenges but also through long-term wellness planning and treatment management. 


According to the National Alliance on Mental Illness (NAMI), effective care coordination can significantly improve treatment outcomes, particularly for those managing mental health conditions. NAMI believes that all people with mental health conditions deserve accessible, affordable and comprehensive health care. NAMI supports the use of coordinated care models that integrate physical and mental health services.  With her dual experience, Fatoumata embodies the best of both worlds—connecting on a personal level while delivering comprehensive, structured care.
