IF YOU ARE THINKING ABOUT SUICIDE OR IF YOU ARE CONSIDERING TAKING ACTIONS THAT MAY CAUSE HARM TO YOU OR TO OTHERS OR IF YOU FEEL THAT YOU OR ANY OTHER PERSON MAY BE IN ANY DANGER OR IF YOU HAVE ANY MEDICAL EMERGENCY, YOU MUST IMMEDIATELY CALL THE EMERGENCY SERVICE NUMBER AND NOTIFY THE RELEVANT AUTHORITIES. OUR PLATFORM AND SERVICES ARE NOT DESIGNED TO TREAT MEDICAL EMERGENCIES AND OUR PROVIDERS CANNOT PROVIDE ADEQUATE ASSISTANCE IN SUCH CASES.
Terms and Conditions

These are the Terms and Conditions governing the use of this Service and the agreement that operates between You and the Company. These Terms and Conditions set out the rights and obligations of all users regarding the use of the Service.
Your access to and use of the Service is conditioned on Your acceptance of and compliance with these Terms and Conditions. These Terms and Conditions apply to all visitors, users, and others who access or use the Service.
By accessing or using the Service, You agree to be bound by these Terms and Conditions. If You disagree with any part of these Terms and Conditions then You may not access the Service.
You represent that you are over the age of 18 or the legal guardian of the minor for whom you are obtaining services.
Your access to and use of the Service is also conditioned on Your acceptance of and compliance with the Privacy Policy of the Company. Our Privacy Policy describes Our policies and procedures on the collection, use, and disclosure of Your personal information when You use the Application or the Website and tells You about Your privacy rights and how the law protects You. Please read Our Privacy Policy carefully before using Our Service.

The Providers and Services Provided

As a CARF accredited company, we require that every provider on our platform be board certified and licensed in their respective state. PM MyWay providers can offer services virtually, in-person at one of our Preventive Measures offices, or a combination of both depending on their scheduling. Each provider’s appointment type can be viewed on the Our Providers page of the Preventive Measures website.
In agreeing with these terms, you acknowledge that you have the choice of choosing your preferred provider. As well as discussing appointment locations/types, available times, cultural and linguistic preferences, and specialties at any time. You also acknowledge that your selection of a service provider should be based on your identified needs, preference, and provider availability.

Any link (including a hyperlink, button, or referral device of any kind) used in the Service is provided for your use and convenience. For quality of services, Preventive Measures may request you to link to third-party websites. The appearance of a link does not constitute an endorsement, recommendation, or certification by Preventive Measures, nor should the presence of a link in any way be construed as a suggestion that any third-party website has any relationship to Preventive Measures. Preventive Measures does not endorse the content on any third-party websites. Preventive Measures is not responsible for the content of linked third-party websites or third-party advertisements, and does not make any representations regarding its content or accuracy. Preventive Measures does not knowingly link to websites that may infringe on valid and existing trademarks, service marks, copyrights, or patents.
Termination
Preventive Measures may suspend or terminate your access to the Service at any time, for any reason, or for no reason at all. Preventive Measures has the right (but not the obligation) to refuse to provide access to the Service to any person, agency, or organization at any time, for any reason, or for no reason at all, in our sole discretion. Preventive Measures reserves the right to change, suspend, or discontinue all or part of the Service, temporarily or permanently, without prior notice.
Intellectual Property
All of the content available on or through the Service is the property of Preventive Measures or its licensors and is protected by copyright, trademark, patent, trade secret, and other intellectual property law. You agree not to reproduce, retransmit, distribute, disseminate, sell, publish, broadcast, or circulate the content received through the Service to any third party. All software and accompanying documentation made available for download from the Service is the copyrighted work of Preventive Measures or its licensors. Any copy made of information obtained through the Service must include all applicable copyright notices.
“Preventive Measures” and “Unlimited Messaging Therapy” are copyright terms. All rights reserved. Subject to the terms of this Agreement, Preventive Measures hereby grants you a limited, revocable, non-transferable, and non-exclusive license to use the software, network facilities, content, and documentation on and in the Service to the extent, and only to the extent, necessary to access and use the Service.
The license granted herein does not permit you, and you agree not to: (i) modify, translate, reverse engineer, disassemble, decompile or create derivative works of the Service or allow a third party, whether directly or indirectly (including, but not limited to the direct or indirect use of wizards, agents, bots, or other utilities), to modify, translate, reverse engineer, disassemble, decompile or create derivative works of the Service; or (ii) transfer, distribute, sell, lease, rent, disclose or provide access to the Service to any third party or use the Service to provide service bureau, time-sharing or other services to third parties.

Notices
We may provide notices to you regarding this agreement, appointment reminders, and other communication that we deem necessary for your care. Notices and reminders may come via email, text, or telephone.
Multiple appointment reminders may be sent prior to your scheduled appointment for either the confirmation or cancellation of your next set appointment. Reminders will cease once your appointment is either confirmed or canceled. Appointments that are canceled within 24 hours of set appointment time may be subject to a $25 fee. Missed appointments without any prior communication with a PM representative may be subject to the same fee. Appointments can be canceled via text or by calling the Preventive Measures office, during business hours, respective to your location.

Billing and Payment
By agreeing to these terms, you agree to pay all fees and charges associated with your Account on a timely basis and according to the fees schedule, the terms, and the rates as published. By providing us with your payment information you authorize us to bill and charge you through that payment means and you agree to maintain valid payment information in your account information. Charges to your account will be processed by the close of business on the day of your scheduled appointment. If your appointment is scheduled for after business hours, your payment will be processed the following day. If your appointment is scheduled during the weekend, your payment will be processed the following Monday.
